The muscles of pelvic floor (PF) are some of the most overlooked and misunderstood muscles in the human body. “By virtue of being deep inside the body, [the PF muscles] are frequently forgotten when considering overall fitness. Yet for everyone (and for women in particular) they can easily become dysfunctional” (Herbert, 2010, p. 27). Unfortunately, the dysfunction of the PF can have life-altering side effects. The muscles and fascia that compose the PF support all the internal organs, provide urinary and fecal continence, play a role in sexual function, and provide stability for the spine (Bø & Sundgot-Borgen, 2010; Mohseni-Bandpei, Rahmani, Behtash, & Karimloo, 2011). Without a strong PF, one may experience back pain, incontinence, and pelvic prolapse.

Although the Kegel exercises were developed in 1948 (Borello-France, Downey, Zyczynski, & Rause, 2008), the Pilates Method has been one of the only exercise programs to systematically incorporate PF activation into its protocol. Furthermore, Pilates also focuses on proper alignment of the lumbar spine and coactivation of the transverse abdominis and the PF. In one simple Pilates maneuver called cocontraction, the lumbar spine is aligned, and the transverse abdominis and PF are fired (flexed). The Pilates Method and its focus on PF activation, spinal alignment, and abdominal activation can help to prevent or reverse back pain, incontinence, and pelvic organ prolapse in women.
